To apply for an equipment grant from the Better Life Foundation WA, please download an application form and return via the submission form below.
The application form has two parts: Part One is to be filled in by the applicant, parent, spouse or guardian; Part Two is to be filled in by the treating health professional.
The Better Life Foundation WA will accept applications for equipment grants during the following dates for the 2018 year:
Round 1
1st January 2018 (applications open) to 28th February 2018 (applications close)
Round 2
1st April 2018 (applications open) to 31st May 2018 (applications close)
Round 3
1st July 2018 (applications open) to 31st August 2018 (applications close)
Round 4
1st October 2018 (applications open) to 30th November 2018 (applications close)
It is expected that all applicants will be notified on the outcome of their application 4 to 6 weeks after the closing date of each round.
